No Service Agreement will go into effect until Cochlear receives a completed and signed Service Agreement application form with full payment. Payment and application form must be received by Cochlear no later than June 30, 2009. You may not elect monthly payments if you wish to take advantage of this offer. If your sound processor is currently under the original 3-year warranty, it must expire on or before December 31, 2009 to take advantage of this offer. The initial term of the Service Agreement is 12 months. Following the initial term, the agreement will automatically renew for successive 12-month terms at the newly established annual fee. The Service Agreement cannot be canceled during the initial 12-month term. Following the initial 12-month term, the agreement is only cancelable if the recipient provides written notice not to renew at least 30 days in advance of term expiration. Cochlear will not refund amounts paid toward Service Agreements. Offer is valid on direct-to-recipient sales only. Offer may not be used with any other offers, contracts, promotions, or credits. Service Agreement pricing is available in the U.S. and Canada only, and is subject to applicable taxes and additional terms and conditions.
